Name Change By Deed Poll - Changing your Name
Name changes using a deed poll, or deed change of name, are legal documents used in the United Kingdom for name changes. There are no laws stating the deed poll must be recorded in any court or clerk's office. However, some people choose to have the deed poll registered with the Supreme Court where they eventually make it to the British National Archives.

Three Components to a Deed Poll
When you fill out a deed poll, you must include three components. They include:
- Current Name: Oath vowing you will no longer use your current name. Be sure to list your current name on the form.
- New Name: Your full, new name and an oath stating you will be using this new name at all times in the future.
- Request: Statement that you require everyone having any dealings with you to address you by your new name and your new name only.
Laws Regarding the Use of Deed Polls
Anyone using a deed poll for a change of name must be 16 years or older. Parents and guardians of minors must request a deed poll name change for children younger than 16 and all legal parents or guardians must agree with the information on the deed poll and provide their signature.
