Gather the Required Paperwork
The first thing you need to do is gather your paperwork and fill out a replacement driver's license form. Most DMV offices will want to see proof of your name change, such as a court notice or a marriage certificate.
Expect to provide:
- Check for the replacement license fee (they vary from state to state, in Vermont the fee is $15, but it is $4 in Arizona.) Bring your checkbook, some DMV offices will not accept cash.
- Paperwork listing your new name.
- Proof of who you are and your current residential status (piece of mail usually works fine).
- Replacement license form filled out completely in black ink.
- Your current driver's license.
If you want a replacement photo driver's license, some states require them while others do not, you will pay an additional fee for the photo. In many states that fee is $5.
Handling a Name Change by Mail
If you have a hard time getting to a DMV office, see if you can do it by mail. Some states do allow this. You will need to submit certified copies of the required paperwork, so make sure it will be returned or pay the expense to get duplicate forms so that you are not giving up your originals.
Don't forget to include a check for payment.
