Why It's Important to do a Social Security Card Name Change
If the Social Security Administration has incorrect information, it can mess up your tax records with the IRS and prevent social security tax payments from reaching your account. If you get married, divorced or change your name for any other reason, it is your responsibility to inform the Social Security Administration of this change.
Printing Out Form SS-5
Your social security name change form must be printed on standard, 8.5 by 11 inch white paper. The form must be printed in dark blue or black ink only, no other color is acceptable. Those living overseas should use A4 paper sized 8.25 by 11.7 inches. Thick paper, oversized paper and patterned stationery are not accepted.
Filling Out the SS-5
The SS-5 form is used to apply for a first time social security card or for name changes. The form is self-explanatory. You'll need to list the names and social security numbers for your parents, your residential information, citizenship and date of birth. Under name, you would list your new name on top and then your birth name in the “Other Names Used” information below.
When you reach question 10, you must check the box stating you've had a social security card issued to you. Fill out the information it requires including your social security number and name shows on the social security card.
Submitting Your Social Security Name Change Paperwork
You can mail your SS-5 to the closest office or bring it in person. You can call the Social Security Administration (1-800-772-1213) to find the location nearest you. Make sure you ask for office hours. Many social security offices are only open from 9:00 a.m. to 4:00 p.m. making it difficult to visit the offices if you work a more traditional 8:00 a.m. to 5:00 p.m. schedule.
The form SS-5 must have certified copies of your original marriage certificate, court order for the name change, divorce decree. In addition, you must provide a certified copy of a form stating your old name such as your birth certificate and a photo ID. You can also send originals and they will be returned.
