A living will form is a pre-drafted document that sets forth a person's wishes not to prolong life through artificial means and is available from multiple sources such as a free living will form, a living will sample, a living will document or as an example of living will.
There is software available that allows you to prepare a living will document. Typically, the software takes you through a step-by-step process to prepare a living will document that is tailored to your specific needs. Collateral document forms, such as a health care directive or health care power of attorney, are also included. An example of a living will is usually state specific and conforms to the laws and legal requirements of that state.
A living will form is a starting point for documenting your medical wishes should you become mentally or physically incapacitated. There should be incorporated into the living will form, or as an accompanying document, a reasonably detailed medical directive providing a physician or hospital with instructions regarding life support, possible surgery or other general and specific medical treatments in the event of your mental or physical incapacity. It is also useful to appoint a medical agent, through a medical power of attorney, to act as an intermediary between your documented wishes and medical care providers to deal with unanticipated medical situations.
In filling out a living will form, or an advance health care directive as some states call it, there are a number of substantive categories that should be considered. A statement of preferences sets out those medical procedures and equipment that are acceptable, or not acceptable, in terms of life sustaining methods. The designation of a health care agent is the appointment of the person who will make health care decisions for you in the event you are unable to communicate your wishes after an injury or life threatening event. The living will document should include the health care agent's name, address and telephone numbers. A conservator is a court appointed person authorized to manage your personal affairs if you become legally incompetent. The living will document can either designate your health care agent, or someone else, as your conservator. If you decide to donate body parts upon your death, the living will form can specify which body parts are to be donated. Witnesses are required to observe your signature of the living will document. The names and addresses of the witnesses should be provided.
