A living will form is a pre-drafted document that sets forth a person's wishes not to prolong life through artificial means and is available from multiple sources such as a free living will form, a living will sample, a living will document or as an example of living will.

A statement of preferences sets out those medical procedures and equipment that are acceptable, or not acceptable, in terms of life sustaining methods. The designation of a health care agent is the appointment of the person who will make health care decisions for you in the event you are unable to communicate your wishes after an injury or life threatening event. The living will document should include the health care agent's name, address and telephone numbers. A conservator is a court appointed person authorized to manage your personal affairs if you become legally incompetent. The living will document can either designate your health care agent, or someone else, as your conservator. If you decide to donate body parts upon your death, the living will form can specify which body parts are to be donated. Witnesses are required to observe your signature of the living will document. The names and addresses of the witnesses should be provided.
